2/14/2024 EnchantaVaughn Approved Behavior: QMHP-C met with Client for in-home services and to check on how she was doing since the last visit. Client was present in the home during session. Client communicated she was in a good mood. Client was witnessed adhering to the rules and regulations in the home and giving mom assistance with anything she needed done around the house. Behavior: Client was applauded for her teamwork skills and presenting excellent behaviors. Client responded with positive gestures and continued following along with each prompt. Client demonstrates excellent conduct in her seat and reframing from moving around and making loud noises Intervention: QMHP-C went to the home to meet with Client individually. QMHP-C engaged in conversation with Client about her day. QMHP-C actively listened to Client verbalize that she was agitated today and the reasons to her agitation. Instruction, modeling and role-playing techniques were used to help Client’s general communication skills. QMHP-C taught Client effective problem-solving skills (i.e. identifying the problem, brainstorm alternate solutions, select an option, implement a course of action, and evaluate). QMHP-C informed Client that when she is agitated to refrain from taking it out on people and communicate to others, so they know exactly how she is feeling. QMHP-C listened to mother verbalize how Client became agitated when she didn’t get what she wanted. QMHP-C encouraged mother to allow Client to express herself in an age appropriate manner and to redirect when needed. Intervention: QMHP-C modeled pro-social behaviors for Client to strengthen her understanding of them. QMHP-C engaged Client in a role reversal exercise and required Client to think about her goals for the future. QMHP-C commended Client on utilizing coping strategies to increase her understanding of feelings and emotions. QMHP-C intervened to prevent the negative behaviors from escalating. QMHP-C physically redirected Client to an alternative activity. QMHP-C provided guidance on alternative behaviors and clear instruction on the alternative behavior. QMHP-C provided positive feedback when Client engaged in the more appropriate behavior. QMHP-C will collaborate with mother regarding behaviors and performance in the home that is a result of the lockdown and what things she is doing to keep Client on track with her service plan. Response: Client stated that she doesn’t intend on being negative and agrees she need to work on controlling and channeling her anger, as well as, listening and communicating her feelings better. Client displayed management of individualized behaviors. Client has not overreacted with anger to minor frustrations or irritants. Client responded in a pleasant manner acknowledging the positive reinforcement that QMHP-C provided. Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD. Client was able to reevaluate herself and display a more positive attitude toward the authority. Response: Client demonstrated expectations and responded well to feedback. Client reported a decreased stress level due to her modulation of social contact, and the success of this was reviewed. Client still struggles with coping with stressors and requires services but have shown some improvement in this area. Client is making progress with expressing her feelings and showing others respect and courtesy by talking with appropriate tone, initiating conversation, and waiting for her turn to talk before blurting out loud. Client said she loves coloring activities and utilizes this to relax and stay focused in the home. Client smiled at QMHP-C and said thank you.
